One hundred percent parent/care giver participation is the cornerstone of our approach to using play as a means to learning. This, of course, extends to all caregivers because we all play an important role in our children’s lives!

By completing the enrollment form, you agree that you will be within “touching distance” of your child at all times and that you hold Sensational Kids (a division of Pediatric Physical Therapy & Services, Inc.) free of all liability or responsibility for any injury or harm to yourself or your child while on PPT&S Inc. property or while participating in any activities associated with Sensational Kids classes.

You also give us permission to create still or video images of your child for the purpose of advertising, for use on any Pediatric Services website, for instructional purposes and collaboration with other pediatric professionals, and for community awareness programs. It is the policy of Pediatric Services to use only first names and not include information that would easily identify a family. If you prefer not to have your child's image used for any particular project by Pediatric Serivces, you will have at least 24 hours during which you may opt out of the project by providing written notice to us.

Each month's class schedule consists of 4 classes. Make up sessions will be held the 5th week of the month when time and space are available. Class sizes are limited to ensure the continuity of age-appropriate interactions, safety, and small class size.

There is a one-time registration fee of $25 and a monthly class fee of $40.

This agreement remains in effect unless revoked in writing by either party.
